Introduction and Overview
The Sickle Cell Foundation of Georgia, Inc. (SCFG) is a non-profit organization based in Atlanta, GA providing programs and services to individuals living with sickle cell disease (SCD) throughout the state. Our mission is to empower individuals with SCD to live healthier, fuller lives and reduce the burden of this disease on affected communities. According to the Georgia Sickle Cell Data Collection Program (SCDC), there are approximately 16,000 cases of SCD in the state, 6,720 are children and adolescents up to 19 years of age. Across the state around 33% of individuals with SCD do not have access to informed primary care and 12% do not have access to specialty care. Individuals with SCD and their families face a myriad of challenges with the health care system, schools, employment, and everyday life. The Georgia Community Connections project was established to assist individuals and their families with some of the challenges and to improve the health outcomes and improve the quality of life of individuals living with sickle cell disease (SCD) in the state of Georgia.
The intent of this solicitation is to continue building a network to increase local access to resources and services across the state. Non-profit organizations are key partners in addressing social and health care needs; providing access to community resources; promoting health; and improving the overall health and well-being of individuals served. The purpose is to build the capacity of community-based organizations across the state to provide local services to individuals living with SCD and their families and establish a care continuum across the state.

Non-profit organizations located in Georgia providing services to individuals with SCD and their families.
How Can Funding be Used?
Funding may be used to build organizational capacity to provide services for individuals with SCD and their families. Services may include, but not be limited to, identifying individuals with SCD and connecting them to care coordination services in your area; educating individuals and their families; supporting or establishing transition services in your local communities; providing family support services; etc. Capacity building is whatever is needed to bring your organization to the next level of operational, programmatic, financial, or organizational maturity, so it may more effectively and efficiently advance its mission into the future.
